    Sports Physical Forms 

    A physical form dated after April 15 of the preceding school year MUST BE ON FILE at Davis for 7th and 8th grade sports PRIOR to tryouts. Physical forms can be picked up in the main office at Davis.

    For 9th grade/Freshman sports, same as above, but on file at Stevenson and families will need to fill out the Participation Form
